This potion usually comes in two parts, the aging potion and a de-aging potion. This dangerous potion can be used to temporarily age the imbiber. You must make a Constitution saving throw DC 16. On a success you age by 2 categories. On a fail, you age by 3 categories. Apply the ability score adjustments after aging. If you age past venerable you must make a secondary Constitution saving throw DC 16. On a success your age category is venerable. On a failed save you die instantly. The effects of an aging potion last for 24 hours, or until a de-aging potion is consumed.

You create an invisible sensor within 100 feet in a location familiar to you (a place you have visited or seen before) or in an obvious location that is unfamiliar to you (such as behind a door, around a corner, or in a grove of trees). The sensor remains in place for the duration, and it can’t be attacked or otherwise interacted with.
A creature that can see the sensor (such as a creature benefiting from see invisibility or truesight) sees a luminous, intangible orb about the size of your fist.
For the duration you can hear everything in the area as if you were standing there. The effect lasts for the duration as long as you remain on the same plane of existence.
You create an eye that hovers in the border ethereal for the duration. You mentally receive visual information from the eye, which has vision out to 30 feet. The eye can look in every direction.
As an action, you can move the eye up to 30 feet in any direction. There is no limit to how far away from you the eye can move, but it can’t enter another plane of existence. It can pass through any non-magical barriers. See the Border Ethereal entry for more details.
Imbibing the potion enables you to move up, down, and across vertical surfaces and upside down along ceilings, while leaving your hands free. Your climbing speed is equal to your walking speed.
This potion affects the mind of the character so that he believes the liquid is some other potion (whatever type the DM announces or hints at).
For example, the imbiber believes it to be a healing potion, and believes hit points are "restored'' by drinking it, and only realizes after the duration has expired or it is revealed through combat that the potion only caused the imbiber to believe that he was aided.
After drinking this potion, the individual (and everything he's carrying and wearing) diminishes in size. Each dose reduces the individual’s size by half to a minimum of 6 inches (remember, 5e always rounds down). The effects of this potion last for one hour, after which the individual’s size doubles every minute, until restored to full size.
The ESP potion bestows an ability that is the same as the spell description for Detect Thoughts (save DC 13).
A potion of extra-healing restores Hit Dice and grants a number of temporary hit points per type.
